Our practice is dedicated to preserving the health of our patients and providing educational information that can help people take better care of themselves. We offer routine preventative care, as well as diagnosis and treatment for gynecological, obstetrical, and urinary conditions. Our caring and compassionate manner puts patients at ease and contributes to a positive and fruitful experience at each visit. Patient Forms

Our office has electronic medical records. We will mail out a letter with your log in ID and temporary password. We ask you to click on the link on the sidebar to go to DigiChart to fill out your medical history and allow us to better serve your healthcare needs in a timely fashion.
                                     
When you first come to Atlantic OB/GYN, you will need to fill out the following forms. To save you time on the day of your appointment, please click on the following links and print each form. Bring them to your appointment completely filled out, along with your insurance card and be prepared to pay your copay that day. If you are under the age of 18, you will need to have a parent or legal guardian come with you to review and sign all legal documents and financial policies. After that visit, you may come to every other appointment alone until the next calendar year, when you will need to update your information again. All patients will need to fill out these forms every calendar year.

Virginia Beach Office

Driving Directions:
From Norfolk, take 264 eastbound. Get off on Rosemont Road and bear to the right. Follow Rosemont road to Holland Road (approximately 6-8 stoplights). At Holland Road, make a right. The next light is Chimney Hill Parkway. Make a right and you will be turning into our parking lot. We are located in Suite 101.

From the oceanfront take 264 westbound. Get off on Rosemont Road. Make a left onto Rosemont road and follow to Holland Road (approximately 6-8 stoplights). At Holland Road make a right. The next light is Chimney Hill Parkway. Make a right and you will be turning into our parking lot. We are located in Suite 101.

Driving Directions:
Take I64 eastbound towards Chesapeake/Suffolk. Take the Battlefield Blvd. exit southbound. Follow Battlefield Blvd. until you come to the 6th stoplight. This will be Medical Parkway. Make a right onto Medical Parkway. Take the first left, which will be Kingsborough Sq. Go to stop sign. We are the first building on the right after the stop sign. We are located in Suite C.
